About the Business
Disability and Specialised Services Our Disability and Specialised Services (DaSS) Division delivers essential support to South Australians through high-quality NDIS services, sector reform, inclusion-focused policy. We provide direct disability supports through Supported Independent Living, and specialised care through our Disability, Clinical and Quality Services, as well as Aged Care Projects. Our work is strengthened by Corporate and Engagement teams, ensuring strong governance, collaboration and positive outcomes for people we support.

Role Details
Lead safe, timely and person-centred transitions from hospital to home for people living with disability. As Area Manager - Transition to Home you will apply your expertise to shape services and systems that support participants with complex needs to move into the community with dignity, stability and confidence. You'll empower teams, strengthen partnerships and ensure high-quality compliant service delivery that supports better outcomes for participants, families and carers across South Australia. You will provide leadership across the Transition to Home service sites, ensuring the delivery of high-quality compliant transition services aligned with NDIS Practice Standards and organisational priorities. You will lead and support a workforce to strengthen practice quality, respond to complex participant needs and deliver evidence-based services that support positive outcomes. A key focus of the role is building and maintaining strong internal and external partnerships with participants, families, guardians, hospitals, community providers and government and non-government agencies to ensure coordinated, responsive and person-centred transitions. This is a multi-classified role available at either ASO7, AHP4 or RN3 level. Applicants who meet the requirements for either classification are encouraged to apply. Appointment and remuneration will be determined based on the successful applicant's qualifications, professional background and experience, in accordance with the relevant industrial instrument.
