Overview

The Support at Home Coordinator is responsible for the coordination and daily oversight of services delivered under the Commonwealth Home Support Programme (CHSP) and the Support at Home (SAH) Program, ensuring safe, person‑centred care that supports older people to live well at home and remain connected to their community.

A key focus of the role is to champion and embed wellness and reablement approaches across all service streams, The coordinator ensures that all services are delivered in ways that build capacity, enhance independence, and support clients to achieve their goals.

The position provides leadership to staff to ensure practice is aligned with aged‑care quality standards, evidence‑based reablement principles, and organisational values. Working collaboratively with clients, carers, multidisciplinary teams, and community partners, the coordinator facilitates responsive service delivery, timely review, and positive client outcomes that reflect a strong commitment to optimising function, choice, and quality of life.
