Let's get straight to the details...
As the core function of our business, in this role you will deliver quality, coordinated, efficient and evidence-based case management services to the participants of the Lifetime Support Scheme (LSS).
To do this you will assess, coordinate, review and monitor treatment, care and support for participants with serious injuries that may include brain and spinal cord injuries, amputations, burns and blindness. You will also implement and manage the treatment, care, and support services participants receive, and manage the related administration.
Utilising a person-centred approach, you will work closely with LSS participants to develop comprehensive, high quality and holistic plans, which respond to and support their unique and changing needs throughout the different phases of their recovery journey.

You may also be responsible for assisting with the supervision and coaching of Service Planners and students within the team, depending on your level of experience.

The experience we are looking for...
To be successful in our position you will have:
- Demonstrated experience working with people with serious injuries including assessing, coordinating, and reviewing their health support needs, using a person-centred approach.
- An understanding of the needs of individuals with a disability, as well as culturally and linguistically diverse backgrounds, including the ability to adapt your approach and communication style based on a participant's needs.
- The ability of how to exercise initiative, judgment, and delegated authority to make informed decisions in selecting appropriate assessment and treatment methods.
- For an AHP2, a relevant degree, or equivalent qualification, which entitles registration as an allied health professional, such as physiotherapist, occupational therapist, psychologist, developmental educator or social worker.
- For an ASO5, a relevant degree, or equivalent qualification, in personal injury/disability services such as vocational rehabilitation, personal injury management, nursing, etc

Position Conditions: Full-time, Ongoing and Fixed-term Opportunities. Requests for Part-time will be considered.
Remuneration: ASO5 ($89,007 - $98,991 increasing in July) AHP2 ($94,997 -$110,052) + Superannuation.
Special Conditions
- Appointment will be subject to the provision of a valid Working with Children Check and a satisfactory National Police Check (NPC) that is no more than six months old.
- Some out of hours work may be required.
- Travel to health and community facilities, participants' homes, intrastate and interstate may be required.
- Must hold a current Australian driver's licence.
