Weekend Occupational Therapist - Bridge the Gap Between Hospital and Home
Join SA Health as a Weekend Occupational Therapist in our Transition Care unit at Flinders Medical Centre, where you will support patients during their crucial journey from hospital to home. About the role:
Reporting to the Transition Services Manager Manager, this role is responsible for the implementation and management of Transition Services delivery model. The incumbent will be responsible for OT clinical intervention and discharge planning for inpatients of Transition Services wards.
Duties for the role will include:
- Triaging new admissions and existing caseload for OT assessment and intervention,
- Utilising specialised therapy skills to provide OT intervention and case management to patients,
- Actively collaborate in early and timely multidisciplinary discharge planning,
- Developing and documenting patient-focussed SMART goals in active collaboration with patient, family and /or carers,
- Developing and building on existing relationships within the acute sector,
- Developing and building on relationships with accredited service providers
About you:
- A degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia, Australian Health Practitioner Regulation Agency (AHPRA),
- Eligibility for membership of Occupational Therapy Australia (OTA),
- Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs,
- Ability to contribute to plans, develop, implement and evaluate community oriented, holistic therapeutic interventions appropriate for older people and their families,
- Effective liaison, goal setting and negotiation skills and ability to work in a flexible manner,
- Sound skills in building rapport and communicating with a wide range older adults, their carers /family, referrers, hospital staff and service providers,
- Strong clinical Occupational Therapy experience in working with older people and restorative programs in subacute and/or community settings, with age-appropriate interventions and clinical decision making,
- Strong experience in patient-focussed, holistic SMART goal setting,
- Experience in the supervision of Occupational Therapy staff and Allied Health Assistants
