Overview The Case Manager, provides high-quality home-based care to Commonwealth Home Support Program (CHSP) consumers, based on the philosophy of supporting individuals to remain living at home; and to enable consumers to have choice and flexibility in the way their aged care and support is provided at home. The Case Manager will work with consumers, their family, advocate, or carer to identify goals that will form the basis of the Care plan. They will also under guidance utilise their professional judgement to design flexible and appropriate solutions and tailor service delivery to meet the consumer’s unique needs within the boundaries of the program eligibility targets and agreed risk parameters.
