About The RoleJoin Country Health Connect as a Home Support Worker and make a meaningful difference by supporting older people, people living with disability and other consumers to maintain their independence and continue living safely in their own homes. Working within a Consumer Directed Care model, you will provide practical, person-centred support based on each consumer's individual care plan. Your duties may include general household cleaning, laundry, bed making and meal preparation. Working autonomously in the community, you will build positive relationships with consumers and their families while collaborating with Country Health Connect health professionals to deliver reliable, respectful and high-quality support.
