About The RoleJoin Country Health Connect as a Community Support Worker and make a meaningful difference by supporting older people and people living with disability to maintain their independence and quality of life across the Limestone Coast. Working directly with consumers in their homes and communities, you will provide individualised support in accordance with their care plans. This may include personal care, household assistance, meal preparation, transport, shopping and social support. Working autonomously in the community, you will build positive relationships with consumers and families while collaborating with health professionals to provide safe, responsive and person-centred support. About Country Health ConnectCountry Health Connect provides a broad range of aged care, disability and community-based support services across the Limestone Coast, helping people to remain independent and connected within their own homes and communities. Working within a Consumer Directed Care model, services are tailored to each consumer's individual needs, choices and goals. Community Support Workers play an important role in delivering this support, which may include personal care, household assistance, meal preparation, transport, shopping, social support and assistance with mobility. You will work closely with consumers, their families and health professionals to provide responsive and respectful support that promotes independence, choice and quality of life.
